OFFICIAL RULES

NO PURCHASE OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER FOR A CHANCE TO WIN. A PURCHASE OR PAYMENT OF ANY KIND WILL NOT INCREASE YOUR CHANCES OF WINNING. THE SWEEPSTAKES IS INTENDED FOR PARTICIPANTS OF THE 265 HEALTH & WELFARE FUND PLAN. THE SWEEPSTAKES' GOALS ARE TO PROTECT THE HEALTH OF THE PARTICIPANTS BY INCREASING COVID VACCINATION RATES AND TO INTRODUCE PARTICIPANTS TO THE UNION WELLNESS CENTERS.

Winners will be randomly selected by an un-biased outside third-party.

1. ELIGIBILITY:

The “SHOT FOR SHOT” (“Sweepstakes”) is open to all participants in the SMART Local Union 265 Health and Welfare Fund who receive their vaccine or provide an approved medical exception. It is our intent, that if you are fully vaccinated or have 1 of a 2 shot vaccination cycle on or before January 9, 2022, you would qualify to participate. Employees of any SMART Local 265 Fund, Union officers and staff, as well as Trustees of the affiliated Funds, are not eligible for this raffle.

By participating, winners release and agree to hold harmless Promotion Entities and each of their respective affiliates, directors, employees, officers and agents, including without limitation, their advertising/promotional agencies from any and all liability, injury, loss or damage of any kind, including but not limited to personal injury or death, arising from or in connection with participation in the sweepstakes, or the awarding, receipt, possession, use or misuse of any prize and/or with respect to participation or prize-related activity. Acceptance of a prize shall be construed as and signify prize winner’s agreement and consent that Sponsor may use the prize winner’s name, photograph, likeness, voice, biographical information, prize information, statements for advertising and/or publicity purposes worldwide and in all forms of media now known or hereafter developed, in perpetuity, without further compensation, review, approval or payment, where allowed by law. By participating in this Sweepstakes, entrants agree to be bound by the official rules and the decisions of the Sponsor, which are final and binding in all respects. Sponsor not responsible for any typographical or other error in the printing of the offer or in the administration of the Sweepstakes.

2. SWEEPSTAKES PERIOD:

Sweepstakes begins at 12:00:00 p.m. Central Time on November 9, 2021 and ends at 11:59:59 p.m. Central Time on January 9, 2022 (“Sweepstakes Period”). Sponsors’ marketing company computer is the official time keeping device for this Sweepstakes. It is our intent, that if you are fully vaccinated or have 1 of a 2 shot vaccination cycle on or before January 9, 2022, you would qualify to participate. All entries are timestamped and will not be valid after 11:59:59 p.m. Central Time on January 9, 2022.

3. HOW TO ENTER:

Website: During the Sweepstakes Period, Participants in the SMART Local Union 265 Health and Welfare Fund can enter to win a prize at ShotForShot.org. Participants who visit the site will have the opportunity to upload their vaccine card along with registrant information. Each participant can only enter the Sweepstakes once. Participants will have to match their credentials against a current member directory to have the ability to enter. When the participant has uploaded their vaccine card they are entered to win the Sweepstakes prizes.

For All Entries:

If entering with a mobile phone or other web-enabled device and using your wireless carrier’s network, standard data charges from your wireless carrier may apply. Check with your wireless service provider for details on these and any other applicable charges. Entrants are solely responsible for any such wireless charges. All Entries must be received and recorded during the Sweepstakes Period. No other forms of Entry are valid. Winning entrants will have their vaccine batch number checked for authenticity.

4. SWEEPSTAKES DRAWING:

Winners will be selected in a random drawing to be held on or about the Drawing Date as defined in Rule #2 by Sponsors, whose decisions are final in all matters relating to this Sweepstakes. Odds of winning depend upon the number of eligible entries received for the applicable Entry Period. Non-winning Entries do not carry forward to subsequent Entry Period drawings. If any potential prize winner is found to be ineligible, or if he or she has not complied with these official rules or declines the prize for any reason prior to award, such prize will be forfeited and an alternate prize winner may be selected.

5. WINNER NOTIFICATION:

All prizes will be awarded. Potential winners will be notified via email and/or phone and will be required to respond to the notification within forty-eight (48) hours indicating whether he/she can accept the prize. If a potential winner does not respond to the notification within the forty-eight (48) hour time period, prize will be forfeited and an alternate potential winner will be selected. Any alternate potential winner selected will also be required to respond to the notification within the time frame stated above.

Potential winners may be required to complete, sign and return an affidavit of eligibility and liability and, unless prohibited by law, publicity release to Sponsors within forty-eight (48) hours of prize acceptance.

In the event of noncompliance within any stated time period, the prize will be forfeited and an alternate potential winner will be selected. Any alternate potential winner selected will also be required to adhere to the time periods described herein. Any prize notification or prize returned to the Sponsors or its agencies as undeliverable will result in disqualification and the awarding of that prize to an alternate potential winner.

Sponsors are not responsible for suspended or discontinued Internet, wireless, or land-line phone service or a change in an entrant’s email, phone number or mailing address which may result in a potential winner not receiving initial prize notification or his/her prize information.

Winners shall be responsible for any and all federal, state, and local taxes, fees, assessments and like charges due in conjunction with acquiring a prize, if any. Winners are advised to consult his or her own tax professional to determine the extent of any such obligations. Prizes with a value in excess of $5,000 will be subject to federal and state withholding requirements.

6. PRIZE DETAILS:

First prize is the choice of a Harley-Davidson motorcycle or a new fishing boat or a new side by side ATV or $25,000 (cash value of these prizes). Second prize is $10,000. Third prize is $5,000. Remaining prizes will be 100+ Apple Watches.

7. DISPUTE RESOLUTION:

All issues and questions concerning the construction, validity, interpretation and enforceability of the Sweepstakes, or the rights and obligations of the Entrant and Sponsors in connection with the Sweepstakes, or any claim or dispute that has arisen or may arise between you and Sponsors, shall be governed by, and construed in accordance with, the laws of the State of Illinois without giving effect to any choice of law or conflict of law rules.

8. PRIVACY POLICY:

See Sponsors’ privacy policy for details of Sponsors’ policy regarding the use of personal information collected in connection with this Sweepstakes. If you are verified as a prize winner, your first name, last initial, city and state will be included in a publicly available list of winners.
